The human brain contains a map of our body that manifests as an organized pattern of brain activity. This structure, known as the somatotopic map, specifically refers to how tactile sensations relate to different areas of the brain. However, we still do not fully understand how this map is organized and why it is so precisely structured.
Changes in the Somatotopic Map After Nerve Repair
A new study has examined the changes in the brain map of the hand after surgical repair of hand nerves. The results indicate that the brain map of individuals who underwent nerve repair has experienced changes, and these changes may positively impact rehabilitation.

This map allows us to understand how touching different fingers creates distinct patterns of brain activity. For example, stimulating the thumb or little finger activates specific areas of the primary sensory cortex (S1). This organized spatial arrangement represents the somatotopic map, which has a history dating back to the 1930s.
Implications of Brain Map Changes for Rehabilitation
Peripheral nerves that connect the brain and spinal cord to other parts of the body have an astonishing ability to regenerate. However, the regeneration process is not precisely directed, and new nerves may connect to different areas of the hand. This could mean that when a person touches their index finger, they may feel the sensation in their middle finger.
Our study used functional MRI to examine the brain maps of 21 individuals who underwent hand nerve repair and compared them with 30 individuals without nerve damage. The results showed that the natural organization of responses to fingers in individuals with repaired nerves has changed and is more variable.
These map changes are a result of new nerve connections in the hand, but they may also indicate changes in the brain itself. The findings suggest that successful recovery may not require the reconstruction of the brain's original map, and the nervous system may effectively utilize the new relationships between the hand and the brain.
